PERMANENT Part-Time, Cobourg & Port Hope

This position will be shared between our two Northumberland County Long-Term Care Homes, Extendicare Cobourg and Extendicare Port Hope. Both of our Homes are conveniently located just off Hwy 401.

What You’ll Be Doing
- Consult with and advise the Dietary Manager and Director of Care concerning nutrition and hydration program needs through performing nutritional assessment using RAI MDS 2.0, nutritional care planning, implementation and follow-up for residents by involving the resident, family members and care team.
- Registered Dietitian, eligible for membership with the College of Dietitians of Ontario
- Ideally, 2-3- years experience in a long-term care setting with extensive knowledge of geriatric nutrition
- Proficiency and Experience with RAI MDS 2.0, familiarity with software PointClickCare is an asset
