Are you looking for an opportunity to utilize your Registered Nurse (RN) training and experience to make a real impact on the lives of Albertans? The Frequent Visitor to Emergency Department Program is currently searching for an RN to join our team. This position will provide clinical input and guidance for an interdisciplinary team to improve care for patients who present frequently to the Emergency Department (ED) with complex medical, mental health and social needs. This role will champion coordinated, patient-centered approaches that reduce repeat presentations, improve outcomes, and strengthen integration between acute, community, and primary care sectors. The position will involve working as a collaborative interdisciplinary team to both create individualized care plans which align with evidence-informed, trauma-informed, and best practice strategies, and to identify those patients whose complex needs would be well served by an individualized care plan and standardized care on their ED visits. This position will provide clinical consultation on complex or high-risk cases and build strong relationships with stakeholders outside of the ED and AHS (primary care, home care, recovery Alberta, local shelters, housing providers, etc.). The successful candidate will also participate in data-driven performance improvement and evaluation cycles.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Psychiatric Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): https://connect.nurses.ab.ca/home/learning-and-development/learning-modules NextGenU Health Assessment Resources Certification: https://courses.nextgenu.org/course/view.php?id=271.

Registered member in good standing with the College of Registered Nurses of Alberta (CRNA). Minimum 5 years of clinical experience in an Emergency Department. Experience with complex case management and care planning. Excellent written and verbal communication skills. Strong understanding of complex patient populations including social determinants of health, mental health, intergenerational trauma, substance use, and chronic disease.
Preferred Qualifications:
Strong skills in Connect Care including system navigation, documentation, care planning and communication tools. Experience working with community providers and marginalized populations. Experience with Tableau in making data informed decisions. Project Management and ProSci Change Management skills. Comfortable with MS Office (PowerPoint and Excel).
