Follow-Up Support Worker at E4C

As a Follow-Up Support Worker (FSW) within e4c’s Intensive Case Management Team, you will support individuals and families transitioning from homelessness into permanent housing. Using the Critical Time Intervention (CTI) model, you will build strong, trust-based relationships with participants, helping them develop individualized action plans, build life skills, connect with community resources, and establish networks that promote long-term housing stability and independence. Your work will ensure that housing becomes a foundation for participants’ overall well-being and success.

Key Responsibilities

Housing Follow-Up Support

  • Apply Housing First principles to help secure and maintain stable housing for participants.
  • Promote e4c’s Intensive Case Management services to property managers and landlords.
  • Provide trauma-informed, person-centered, and harm reduction-based support.
  • Conduct assessments and referrals to identify participants’ strengths and barriers.
  • Assist with funding applications and navigation of community resources.
  • Coach participants on building healthy relationships with landlords and service providers.
  • Conduct regular community visits, including participant transportation to appointments and community activities.

Case Management

  • Build and maintain supportive relationships through the CTI model.
  • Co-develop individualized goals and action plans for maintaining housing stability.
  • Support participants in accessing income supports, healthcare, and vocational services.
  • Facilitate community connections and prepare participants for successful program exits.
  • Accurately collect and input participant data for reporting and evaluation purposes.

What You Bring To The Team

  • Diploma or Degree in Community Support Work, Social Work, Sociology, Education, or a related field.
  • Minimum 1 year of experience working with marginalized, high-risk, or vulnerable populations.
  • Previous experience in the social services sector and/or working with Indigenous communities.
  • Strong understanding of harm reduction, trauma, mental health, addiction, sexual exploitation, and homelessness.
  • Demonstrated ability to de-escalate crisis situations involving individuals under stress or influence.
  • A valid driver’s license and access to reliable transportation are required.
