Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is pleased to partner with various agencies in Dufferin, to offer services to the local community at the *new* Dufferin based Homeless and Addictions Recovery Treatment (HART) Hub.
HART Hubs are community-based care hubs designed to address mental health, addiction and housing needs for people experiencing homelessness. They provide integrated care, offering a wide range of services such as primary care, mental health services, addiction care and employment supports. With targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them recover, achieve housing stability, and inclusion.

We're looking for a full-time permanent Navigator to join our team!
As part of a multi-disciplinary team, the Navigator will complete all necessary assessments to better understand client needs, provide appropriate immediate interventions, and develop a plan for additional services as part of a stepped care approach.
Status: Permanent full-time, 37.5 hours per week. Availability between 8am-9pm Monday-Friday, occasional weekend shifts between 8am-8pm. Note, this posting is for an active vacancy.
Salary: $61,201.01 - $72,001.19 annually. (Candidate qualifications will be considered when determining the starting salary)
Location: This position is based out of 30 Centre St., Orangeville . Due to the nature of this program, this role is on-site and/or community based.
What You Will Do:
- Act as a low-barrier first clinical response and front-line resource to the community, including assisting the registration and intake and processing referrals from other sources
- Conduct screening/assessment interviews with clients requesting services
- Develop a client centered case plan in collaboration with the client and any identified support within a stepped-care approach
- Work to establish rapport and build client engagement with available supports
- Assess for risk and provide crisis support; make connection to crisis services when appropriate
- Provide short term task-based interventions or single session 1:1 support for clients who will benefit from supports to alleviate any immediate distress.
- Facilitate and create groups as needed and act as a back-up facilitator.
- Contact with primary care at regular intervals to report clients’ service initiation and communicate interventions and care plans.
- Work collaboratively with Dufferin Bolton Caledon Multi-Disciplinary Team and HART Hub Dufferin Team to support both organizational needs (e.g., volumes across all regions)
- Develop formal and informal links and networks with other community resources
- Coordinate services within the multidisciplinary team at the HART Hub Dufferin
- Make appropriate external referrals as per the client centered case plan
- Keep up to date with knowledge of external community resources
- assess for risk and respond with mitigation strategies, including crisis intervention and de-escalation supports and connecting to crisis services when appropriate

What You Need to Join Our Team:
- Bachelor's Degree or Diploma in Social Work, Sociology or Health Services
- Minimum 2 years of experience in the mental health and addictions sectors with a focus on assessment, brief interventions and short term supports
- Experience in the application of a variety of screening and assessment procedures and tools
- Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence.
- Openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework.
- Willingness to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc.
- Fluency in a second language, including ASL (American Sign Language) is considered an asset.
- Valid Canadian driver’s license and access to an insured automobile in good repair
- A satisfactory Vulnerable Sector Screening (Police Check) report
