Intake Coordinator - Ontario - 6 Month Contract

The Intake Care Access Representative is primarily responsible for ensuring accurate information is in the scheduling system to ensure accurate client service delivery in a professional, confidential, proactive, safe and efficient solution based manner. The Intake Care Access Representative works collaboratively in a team environment and communicates effectively with all members of the team on a regular basis.

Essential Functions

• Monitors and ensures timely processing of incoming client service requests, and requests for changes or updates to client services within contractual obligations.
• Liaise with customer referral sources as required to ensure customer expectations are met
• Responsible for the integrity of information in Alayacare which would involve data entry, updates of client information, schedules and visits.
• Provide relief in Validation team as required
• Knows and adheres to the formal and informal departmental goals, standards and policies and procedures
• Work with an awareness of health and safety and cooperate towards improved health and safety conditions at work
  
  

Hours Of Work

Monday-Friday 8:30am-4:30pm

Contract Position from January 2026- June 2026
