Guaranteed Hours & Stability: Enjoy a 60-hour bi-weekly guaranteed-hours contract with dependable income and participate in the ongoing on-call scheduling of clients.

Area of Work

Successful Candidates Will Be Providing Community Care To Clients Across London, Middlesex County, And Elgin County. Service Areas May Include, But Are Not Limited To

  • London (North, South, East, and West regions)
  • Middlesex County (e.g., Strathroy, Komoka, Ilderton, Dorchester, Lucan, Thorndale, Mount Brydges, Parkhill, Kilworth, Arva)
  • Elgin County (e.g., St. Thomas, Aylmer, Port Stanley, Port Burwell, West Lorne, Dutton, Belmont, Fingal)

Candidates must be comfortable travelling throughout these communities to provide in-home support based on client needs and scheduling requirements.

Availability Requirements

Successful candidates will be required to maintain consistent availability between 8:00 AM and 5:00 PM, Monday through Friday, and must be able to work every other weekend. Flexibility is essential to support urgent client needs, last-minute coverage, and changes in caseload volume.

Responsibilities

  • Delivering care in accordance with clinical standards, organizational policies, and individualized care plans.
  • Maintaining accurate documentation, communicating changes in client condition, and escalating concerns when necessary.
  • Conducting initial home support visits to assess client needs when needed.
  • Providing home support visits to clients as required for coverage for regular staff absences or high-demand periods.
  • Participating in the on-call rotation, responding to after-hours needs within the scope of the role.

All work performed will be compensated at the employee’s nursing rate, in alignment with organizational pay policies.

Travel Expectations

This role involves significant travel across London, Middlesex County, and Elgin County. Candidates must have reliable transportation and be prepared to drive between multiple client homes daily, including rural and semi-rural areas. Mileage reimbursement will be provided in accordance with Bayshore’s travel policies.

Duties And Responsibilities

  • Provide client care by utilizing the skills of observation, assessment, nursing diagnosis, counseling and health teaching; nurse in charge in the setting and, if required, to the immediate Supervisor at the branch office.
  • Provide care according to the established care plan ensuring that, on an ongoing basis, the emotional, spiritual, physical comfort and safety of clients are met to the greatest possible extent; care must be within the scope of practice of a Licensed/Registered Practical Nurse.
  • Perform routine nursing duties in accordance with company policies and procedures and Standards of Nursing Practice as defined by the provincial licensing body.
  • Perform specialized nursing procedures for which recognized training and current competency can be demonstrated.
  • Observe and document client conditions and changes following proper standards and in accordance with professional and Bayshore Home Health standards.
  • Understand and implement infection prevention practices.
  • Participate in ongoing internal and/or external continuing education activities.
  • Adhere to Bayshore Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Complete other tasks as requested.

RN level nurses who have graduated from an accredited School of Nursing and hold a current registration with the provincial licensing body.

Current CPR certification required

Valid driver’s license, auto insurance and reliable vehicle
