POSITION OVERVIEW

As a Pharmacist, you will actively participate in patient care rounds, liaise with members of the healthcare team and develop a professional relationship with the patient and their family to formulate and implement a medication treatment plan. You will also obtain and/or incorporate additional prescribing wherever practice allows, document all direct patient care activities in the patient’s healthcare record and participate in drug stewardship and quality improvement initiatives to improve the quality of patient care.


This Pharmacist 1 position is responsible for maintaining their own professional development and participating in the precepting and mentoring of other staff and students, ensuring accurate and efficient dispensing of medication including providing functional supervision for pharmacy operations and performing administrative responsibilities when required.

Key Responsibilities:

  • Provision of direct patient care: Establish and maintain a clinical practice within a multidisciplinary team to proactively manage medication therapy for patients with complex and high-risk medication regimens, in collaboration with other members of the team. Determine the patient’s desired therapeutic outcomes and evaluate alternatives to achieve these outcomes. Provide education and training designed to enhance patient understanding and appropriate use of his/her medications, and to enhance adherence with therapeutic regimens.


  • Coordinate accurate and efficient dispensing of medications: Exercise clinical judgment, reviews and monitor medication orders for accuracy and appropriateness, and make necessary changes through appropriate channels. Verify that medication orders and patient information are accurately entered in pharmacy computer system. Oversee record system for narcotics, controlled drugs and study/investigational drugs, as required by legislation.


  • Responsible for own professional development and supporting the professional development of others: Maintain professional development as required by the Alberta College of Pharmacy. Promote professional development of self and others by attending and presenting at educational rounds, journal club, etc. Serve as a resource to staff in area of responsibility for all clinical pharmacy policies, procedures and practice standards. Precepts students in the undergraduate PharmD, post professional Pharm D and/or residency programs, etc.


  • Perform administrative responsibilities: Document daily workload measurement statistics. Collect clinical benchmarking data as required. Complete all necessary reports including but not limited to: safety reports, adverse drug reaction reports, antimicrobial usage forms, etc. Act as liaison between pharmacy department and program in area of responsibility. Actively participate in and represents pharmacy in various internal and external committees, when required.



What You Bring to the Table:

  • Education: Baccalaureate degree in Pharmacy. Completion of an accredited Canadian Hospital Pharmacy Residency is an asset. Doctor of Pharmacy or other advanced degree is an asset. Equivalencies may be considered.
  • Experience: Previous hospital experience in a similar institution. Experience with the provision of comprehensive pharmaceutical care within the hospital setting. Experience with a multi-disciplinary patient care team. Experience with sterile compounding, TPN computerized medication order entry, and unit dose drug distribution is an asset.
  • Skills: Up to date knowledge of the clinical area. Effective drug information retrieval skills to identify information sources and provide current, accurate information. Medical terminology and abbreviations, generic and brand names, pharmacy laws and regulations, calculations, and dispensary function. Basic computer skills. Effective communication skills, Strong stakeholder engagement and time management skills. Good problem solving and organizational skills. Ability to safeguard Covenant Health information as private and confidential.
  • Required Licensures/Certificates/Professional Membership: Eligible or currently registered with the Alberta College of Pharmacy. Certificate of competency from the Pharmacy Examining Board of Canada. Additional Prescribing Authorization granted by the Alberta College of Pharmacists (required to obtain within 18 months of hire date for all new hires into permanent positions). Authorization to administer drugs by injection granted by the Alberta College of Pharmacists (an asset). Alberta Health and Wellness Practitioner Identification Number (PRAC ID) (required). Completion of an accredited hospital residency program or Pharm D program (an asset). Active membership in the Canadian Society of Hospital Pharmacists (an asset).
