Return of Service: $10,000 to new or recent PSW graduate as of April 1, 2023

In exchange for 12 month full-time commitment

The Home at Last (HAL) program is a free service to support the safe and smooth transition of seniors and clients with special needs from hospital to home. The program provides trained and experienced PSWs to assist in the client’s transition home by assisting in such activities as: meal preparation, personal care and other activities of daily living.

Location: York Region, South Simcoe and surrounding areas

Requirements:

• PSW certificate, or nursing background.
• Minimum 3 years valid Ontario G driver’s license, clean driving record.
• Over age 25 for vehicle insurance to drive CHATS vehicle.
• Current Standard first aid certificate.
• Work rotational weekends.
• Community Experience with seniors.
• Demonstrated strong team skills.
• Demonstrated exceptional customer service.
• Ability to communicate effectively in English (verbal/written).
• Demonstrated strong organizational skills.
• Completion of administrative related reports.
