Neobridge Healthcare is building a talent pool of Nurse Case Managers to support OAT/OTP clinics in Winnipeg. This role is ideal for nurses who are strong in patient engagement, follow-up, navigation, and coordination in a structured outpatient environment. Candidates must be eligible for registration in Manitoba through the College of Registered Nurses of Manitoba (CRNM) (and/or the College of Licensed Practical Nurses of Manitoba (CLPNM), depending on the role requirements).

This role is ideal for nurses who are strong in patient engagement, follow-up, navigation, and coordination in a structured outpatient environment. Key Responsibilities
- Coordinate intake, follow-ups, and ongoing patient engagement (reminders, outreach, continuity planning).
- Complete clinical monitoring per clinic protocols (screening tools, vitals, symptom tracking, treatment adherence support).
- Provide patient education (OAT process, safer use/harm reduction principles as aligned with clinic model, care planning).
- Connect patients to community resources and supports (housing, counseling, primary care, social services).
- Maintain clear EMR documentation and communicate updates to prescribers and the care team.
- Active registration and good standing with CRNM (RN/NP) and/or CLPNM (LPN), as required.
- Case management experience in addictions, mental health, primary care, or community health is a strong asset.
- Excellent organizational skills and ability to build trust with patients in complex situations.
- Experience working with OUD populations, concurrent disorders, or interdisciplinary clinic models.
