Rocky Mountain House has an exciting opportunity for a Registered Nurse in Home Care! Located within the Rocky Mountain House Health Centre, you will work with a dynamic, supportive team and will have the opportunity to make a difference in the lives of people in your community. Reporting to the Care Manager, the Registered Nurse - Case Manager is responsible for directing the care of clients under the home care program. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning and facilitate client and family teaching. Through the development of the care plan, you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. Your experience in providing wound care and knowledge of the wound care principles and products, as well as palliative and home parenteral therapy will be essential to making you successful in this position. The daily assignments could include intake, supportive living case management, as well as case management of community clients in a variety of settings. You may be required to cover other assignments as workload dictates under the home care program as a Case Manager and will be scheduled to work on call as part of your rotation.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications

Physically able to perform all assigned tasks and may involve physical exertion not limited to climbing stairs, walking outside, lifting, pulling, pushing, stooping, and carrying which may involve visits to client homes that are not ideal.

Preferred Qualifications

Minimum of 2 years Home Care experience within the last 5 years of nursing preferred, although, skills and experience relevant to nursing practice in community sector will be considered. Experience with HPT, wound care, client teaching, palliative support. Excellent interpersonal, communication, time management and ability to work in a collaborative team demonstrating adaptability and flexibility.
