Who you are

You have a worked in a health care field for a minimum of 3 years. You have experience in patient flow data/process analysis, chart reviews, meeting facilitation, educating and coaching others, and auditing. You have a demonstrated proficiency level in Microsoft Office applications. Training in LEAN Six Sigma and/or Project Management are considered an asset.

Bachelor's degree in a health affiliated discipline and/or an advanced degree or training in a related field.

Minimum 3 years related experience in a health care field.

Written and spoken competence in English is required at an advanced level.

What you will do

The Patient flow coordinator is responsible for, but not limited to:

  • The patient flow coordinator is responsible for working collaboratively with the different health professionals to improve, maintain compliance and effectiveness of key strategic processes and other flow initiatives.
  • Investigating reasons for discharge delays as well as investigating new flow initiatives, making recommendations and leading implementation of solutions while overseeing the operational validation of those solutions.
  • Monitoring outcomes and effectiveness of solutions improving length of stays, readmission, hospitalization avoidance, etc. and implementing changes to sustain gains and drive further flow improvements.
  • Initiates and leads educational activities to ensure proper implementation of new/improved patient flow practices within Horizon.
