Position Summary
- We are seeking a dedicated and experienced Stroke Nurse Practitioner to join our team at Trillium Health Partners Mississauga Hospital Site which is the regional stroke centre. The successful candidate will play a crucial role in providing comprehensive care to stroke patients, from acute management to rehabilitation and follow-up care. This position requires a collaborative approach, working closely with a multidisciplinary team to ensure the highest standards of care.
- The Regional Stroke Nurse Practitioner (RSNP) is an integrated member of the stroke team and works in collaboration with all professionals on the Comprehensive Stroke Unit. The RSNP will also support the West GTA Stroke Network with education and mentoring.
- Reporting to the Director of the Neuro/MSK, the RSNP is an advanced practice nurse whose role is comprised of five domains of practice including clinical practice, leadership, collaboration, research and consultation.
- Supports the Stroke/Neurology program by performing advanced patient care assessments and coordinating care for patients with complex co-morbidities, ordering appropriate diagnostic tests, pain and symptom management, and patient education in an in-patient setting and in the future, the Regional Stroke Prevention Clinic.
- The RSNP engages in clinical mentoring and/or other academic activities through clinical appointment with an academic institution.
- The RSNP works in a collaborative practice model with physicians, nurses and inter-professional team members to provide quality care to patients and their families to achieve quality clinical outcomes through evidence-based practice.
- The RSNP is committed to continuous learning by attending EVT/Stroke Rounds and participating in NP focused education curriculum provided by the neurology team.
- In partnership with Physician and administrative leadership team develops the policy and practice standards to support one patient experience across the organization.
- Provides a key leadership role in evaluating quality of care and patient outcomes within the Stroke Program, in partnership with physicians and other members of the inter-professional team.
- Identifies quality initiatives that will strengthen practice within the Stroke Program.
- The Nurse Practitioner provides leadership and assists with professional development of team members through mentoring and participating in formal and informal education initiatives.
- The RNSP will work with acute stroke patients in collaboration with stroke neurologist and stroke GIM to:
- 1. Provide case management, navigation and facilitate communication of patient information for effective patient flow and transition throughout the hospital
- 2. Support best practices and evidence-based care for stroke patients in the emergency, diagnostic and interventional radiology, critical care unit, step-down unit, inpatient unit and Comprehensive Stroke Unit (CSU) and triage to appropriate destination.
- 3. Mentor nurses on CSU supporting knowledge translation of Stroke Best Practices creating an environment of excellence.

- Skills: Strong clinical skills in stroke management, excellent communication and interpersonal skills, ability to work effectively in a multidisciplinary team, and proficiency in electronic health records (EHR) systems.
- Education: Minimum Master’s level nurse practitioner preparation required
- Certification: Current registration in the Extended Class with College of Nurses of Ontario (Successful completion of Ontario NP-Adult Extended Class examination) required
- Minimum of 3-5 experience in direct acute stroke focus/neurology patient management required advanced assessment and diagnostic reasoning skills
- Strong problem solving and negotiation skills
- Must be able to practice independently and interdependently
- Demonstrated leadership to advance the practice of nursing and inter-professional practice to achieve established goals
- Excellent communication skills and the ability to develop positive relationships with patients, families, physicians and inter-professional team members
- Demonstrated use of theory and research to enable evidence-based practice
- Experience with research methodology, program planning and evaluation
- Current Basic Cardiac Life Support certification
- Current Advance Cardiac Life Support certification preferred
- Satisfactory performance and reliable attendance required
