What We're Looking For:

  • Qualifications: Master's degree in Social Work, Clinical Counselling or other relevant health related discipline.
  • Experience: At least two (2) years' recent, related experience in a community or hospital psychiatric setting, or in a rapid response crisis stabilization function, or an equivalent combination of education, training and experience will also be considered.

Detailed Overview:

In accordance with Fraser Health's Vision, Mission and Values, the Mental Health & Substance Use program mandates and clinical practice guidelines emphasize recovery and rehabilitation, reducing barriers to re-entry into the system of care, and supporting individuals and families affected by acute/episodic mental illness and substance use disorders. This role focuses on providing intensive individual and group therapy, liaison regarding complex situations, and care coordination support across the care continuum to ensure timely treatment, rapid re-engagement, and access to information.

Responsibilities:

  • Provides treatment and support for complex situations to clients and natural supports, collaborating to develop and implement recovery-oriented treatment options and plans.
  • Delivers short term brief interventions, follow-up, monitoring, and evaluating treatment outcomes as required.
  • Conducts individual and group therapy, and family support, using various interviewing, supportive counseling, and assessment methods.
  • Supports the intake process, assessing client needs through initial screening of referrals, determining priority of care, client eligibility, and type of care required.
  • Arranges hospital admissions when required, including involuntary admissions, by securing background information and making necessary communications.
  • Facilitates client utilization of community services and rehabilitative programs, assisting with housing needs and providing necessary referrals.
  • Coordinates services with police, hospitals, crisis lines, and suicide prevention programs, identifying gaps in service and contributing to a coordinated mental health care system.
  • Facilitates family and lived experience involvement activities, providing education to clients and families based on evidence-based practice.
  • Maintains client records and ensures compliance with regulatory requirements regarding information access and disclosure.
  • Provides advocacy for clients, ensuring basic needs are met through various Ministry programs.
  • Participates in quality improvement initiatives, team planning, and professional development activities.

Qualifications:

Education and Experience

  • Master's Degree in Social Work, Counselling Psychology, or relevant discipline.
  • Two (2) years' experience working with clients and families affected by mental illness and substance use disorders.
  • Registration eligibility with a relevant professional association.
  • Valid driver's license and access to a personal vehicle.

Skills and Abilities

  • Comprehensive knowledge of mental health assessment skills, crisis intervention techniques, and therapy methods.
  • Ability to establish rapport with diverse populations and collaborate effectively with other care providers.
  • Strong communication skills, both verbal and written.
  • Ability to prioritize workload in a dynamic environment.
  • Working knowledge of relevant legislations, including the Mental Health Act.
