Job Description

As a Care Facilitator, you will be accountable for, in collaboration with patients and their families, assessing care needs, determining eligibility for services, and developing individual care and service plans. In this role, you will also engage with patients, caregivers and other health care practitioners.

What You’ll Do

  • Assess and determine patient eligibility and care needs for home care services including personal support, nursing and allied health
  • Process referrals for home care services
  • Complete a clinical, problem-based head-to-toe assessment
  • Confirm the patient’s hospital discharge plan
  • Initiate communication with the patient's primary care provider, and ensure follow up with primary care providers regarding patient updates
  • Advise Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.
  • Inform patients and families of ParaMed policies and scheduling practices, as well as the expected time of care.
  • Maintain current and accurate patient records.
  • If required, provide the first in-home nursing visit within twenty-four (24) hours of hospital discharge for patients with complex/chronic diseases including CHF, COPD, Diabetes or other;
  • Order medical equipment and supplies for initial in-home visits
  • Establish a helping relationship with patients and their families

What You Bring

  • Registered Nurse/Registered Practical Nurse in good standing with the College of Nurses of Ontario;
  • Minimum 3-5 years clinical experience working as a Registered Nurse/Registered Practical Nurse
  • Recent and relevant experience providing direct (hands-on) nursing care. Preference will be given to experience within the last 2-3 years;
  • Superior clinical assessment skills;
  • Ability to provide quality clinical service delivery in conjunction with patients and other stakeholders;
  • Ability to work independently;
  • Strong critical thinking and problem solving skills;
  • Effective interpersonal and communications skills;
  • Advanced oral and written proficiency in English is essential;
  • Excellent organizational skills
  • Demonstrated competency in patient centered care principles and practices.
  • Knowledgeable of community resources available for patients and families.
  • Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
  • Must have valid driver’s license and access to a vehicle, as travel to patient homes will be required
  • Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
  • Knowledgeable in best practice standards for community health care and services.
  • Strong oral and written communication skills
  • Strong independent critical thinking and problem-solving skills.
  • Access to a personal vehicle required
