About the Opportunity
Nova Scotia Health has established a new Care Coordination Centre to improve patient movement coordination throughout healthcare journeys. Utilizing artificial intelligence and predictive analytics, it provides real-time views of access to care, patient flow, and health system resources, generating actionable insights for continuous improvement in healthcare delivery. Located at the Halifax Infirmary site of the QEII Health Sciences Centre, it fosters collaboration using real-time data for better patient access, flow, efficient bed management, and discharge planning.
Reporting to the Director of Patient Access and Flow, the Patient Flow Manager is responsible for facilitating patient placement by overcoming delays in admission, transfer, and discharge. This role involves collaboration with Emergency Departments, Health Service Directors, and other agencies. The role requires decision-making that ensures high standards of patient care and efficient operation of the QEII/DGH in a fast-paced environment.
About You
- Relevant Bachelor’s Degree required; Nursing preferred
- Relevant Master’s Degree an asset
- Minimum three years of progressive administrative experience in healthcare management
- Clinical experience in utilization and patient flow management
- Demonstrated organizational skills in leadership, communication, and problem-solving
- Knowledge of Best Practice Utilization Management and Clinical Flow practices
- Ability to interpret and execute administrative directives
- Knowledge in current trends in healthcare practice
- Effective decision-making competency in patient care management
- Knowledge of change management principles
- Flexibility to work independently and in a team
- Knowledge of Administrative Policy/Procedures
- Leadership role and decision-making ability in demanding environments
- Competencies in other languages preferred
Hours
- Permanent Full Time; 75 hours bi-weekly
- 12 hour shifts; rotating 7 days a week
Compensation and Benefits
$42.94 - $57.05 hourly
