Part Time

Salary Range: $38.80 - $39.19 /hour

This role will provide leadership in the completion of resident assessment and care plans through training and orientation, education and ongoing support to the interdisciplinary team. The successful candidate will also ensure Mount Hope meets all reporting requirements for RAI-MDS, including care documentation processes as this relates directly to funding allocations for long term care.

Essential Qualifications

  • Current registration with the College of Nurses of Ontario as a Registered Practical Nurse
  • Valid CPR certificate
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Demonstrated knowledge in assessment and care planning processes
  • Well-developed oral and written communication skills
  • Well organized and excellent time management skills along with attention to detail
  • Effective problem-solving skills
  • Proficiency in computer skills including experience with Point Click Care software
  • Previous experience in teaching and adult learning
  • Demonstrated knowledge of Ministry of Long-Term Care Standards, compliance and documentation requirements
  • A police record check completed within the last six (6) months from a Canadian Police Information Centre (CPIC), inclusive of vulnerable sector screening, will be required of the successful candidate

Preferred Qualifications

  • At least 3 years' experience in a long-term care environment
  • RAI MDS Coordinator Credentialing Course
  • At least 2 years' work experience with RAI MDS
  • Demonstrated success leading new initiatives

Immunization Requirements

  • Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
  • Provide documentation of the Tuberculosis skin testing
