Adult Planner at Community Living St. Catharines
Community Living St. Catharines is recruiting for the following Position:
Position: Adult Planner, Permanent, Full Time, Posting Date: November 14, 2025
Program: Community Support Services – Adult Services
Community Living St. Catharines is seeking a highly skilled and motivated individual to develop and facilitate person directed plans for the people we support in our Community Support Services.
Qualifications:
- Minimum – Two-year Community College in a related field, related university degree would be an asset
- A solid understanding and passion for person directed planning, that is deeply rooted in the core values and concepts of self-determination, empowerment, choice and control, dignity of risk, accountability, due diligence, inclusion and citizenship
- Experience working successfully with individuals with multiple and complex special needs, who come from diverse cultural and socio-economic backgrounds
- Experience in facilitating and executing planning meetings with a wide range of stakeholders and community partners
- An extensive knowledge of and experience working with a diverse array of community agencies and resources
- Exceptional organizational, verbal, and written communication skills
- Demonstrated strong interpersonal skills with the ability to affect positive outcomes in complex situations that leads to enhanced quality of life
- Exceptional skills in the areas of prioritization, problem-solving and conflict resolution
- Ability to effectively build relationships with the supported person, their family, their network and the local community
- Ability to facilitate community connections in both formal and informal settings
- Ability to facilitate the building of support circles through networking and relationship development.
- Case management experience
- Proven ability to work as part of a team and independently
- Proficiency in MS Office or other related software programs
- Valid driver’s license and minimum of $1 million liability insurance (use of own vehicle is required)
- Successful completion of a Vulnerable Police Check
Hours of Work: 40 hours per week
Must be flexible to work evenings if required
Salary: $27.00 - 29.00 per hour Start Date: To be determined
