About the role

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

Under the direction of the manager of Patient and Family Centred Care, the Patient Discharge Lead is responsible

for a comprehensive evaluation, assessment, development and education of the Patient Orientated Discharge

Summaries and Patient Oriented Education Tools, which help improve patient transition home, patient experience

and decrease likelihood of adverse effects post discharge.

The Patient Discharge Lead will be responsible to establish a comprehensive review and evaluation of the current

tools, review current practice and patient experience results specific to preparedness and understanding for

successful transitions to home including all methodologies of gathering patient experience results (includes themes

from nurse‑led post‑discharge calls, survey results, interviews, care review recommendations etc). The incumbent

will have a clinical background and an expert ability to identify gaps and priorities while engaging various

stakeholders. In collaboration with patients, program leadership teams, clinical nurse specialists, professional staff

and interdisciplinary team, the Lead will assess, adjust and improve PODS/POETS information and patient

understanding (specific to department) and coordinate communication and education requirements across the

organization to support practice implementation.

Using advance expert knowledge, skills and abilities, the Patient Discharge Improvement Lead will build partnerships

with patients, their care partners and stakeholders in the organization and work in collaboration to meet the needs of

each patient, condition and department. The incumbent will perform assessments and audits to measure compliance

to best practices, policies and other specialty requirements. Using evidence and change management

methodologies, the Lead will seek opportunities to introduce evidence based practice changes that support

improvement in patient understanding, experience and outcomes.

The Lead will also be responsible to evaluate project progress, goals and develop various reports, presentations,

data and summaries on process and outcomes. The incumbent will also maintain statistics and metrics for use in

Patient Experience Improvement, be responsible to provide reporting to OHNW and will seek opportunities to align

with the Patient Experience pillar of the Strategic Plan 2026. The Lead will utilize their extensive knowledge base,

clinical experience and project management skills to support all allied health professionals including ward clerks,

medical staff, NP's , residents and learners. The Lead will also ensure project timelines are met. They will assist in

skill development, including clinical, problem solving and critical thinking. They will also be skilled in addressing

potential patient barriers such as communication issues, patient literacy and lack of knowledge in health care and

available resources.

EMPLOYMENT REQUIREMENTS

Education/Experience

  • Masters level preferred. Minimum requirement of baccalaureate Degree in a Clinical Health related field
  • Minimum 4 years' experience in acute Health Care

Skills/Abilities

  • Must provide patient care or services in a manner that is consistent with TBRHSC mission, vision, values and

exemplifies the philosophy of patient and family centred care.

  • Must treat all information concerning patient affairs or illnesses and/or hospital business as confidential, privileged

information, which must never be discussed except in the course of duty.

  • Must be accountable for ensuring the safest patient care and services and for recognizing and reporting safety

incidents or potentially unsafe conditions.

  • Must abide by the Code of Conduct, work to foster an inclusive environment that honours each individual's

uniqueness, and provides a culturally safe experience through respect and compassion for all staff, professional

staff, patients, and community partners

  • Demonstrated high level of self‑initiative and direction
  • Demonstrated leadership and teamwork skills to support, coach, direct, mentor and teach activities to clinical staff

and to promote positive staff relations

  • Demonstrated strong communication skills both oral and written
  • Demonstrated strong interpersonal skills and emotional intelligence to interact effectively with staff, patients, family

supports and community partners

  • Proficient in the use of computer applications such as GroupWise, Microsoft Office, PowerPoint and Excel

Assets

  • Clinical expertise with knowledge of adverse discharge issues and patient experience
  • Knowledge and experience with Program Planning and Development
  • Knowledge and experience with concepts of project management

Salary Range: $39.979 to 47.034
