Position Summary
The Pharmacist is responsible for ensuring that all prescriptions are dispensed accurately and safely while providing pharmaceutical patient care ensuring the highest level of therapeutic appropriateness for residents. This is a hybrid position with the hours to be split between working at the dispensary in Cambridge and remotely from home.

Your Day To Day
- Responsible for ensuring all prescriptions are clinically verified accurately and safely while providing pharmaceutical patient care
- Helping to maintain the medication safety model implemented at our Long Term Care Facilities serviced
- Supports the medication safety model with the automated dispensary cabinets in our facilities
- Communicates with all members of the resident care team while maintaining professionalism by following corporate policies to ensure the highest standard of customer care and quality at all times
- Maintains accurate, organized and complete resident health records ensuring appropriate documentation and continuity of care at all times
- Conduct medication regimen/profile reviews with associated documentation of recommendations
- Participates in Quarterly PAC meetings at the Long Term Care facility
- Follows all corporate regulations, policies and procedures at all times
- Follows all provincial and federal laws, standards and regulations which govern the practice of pharmacy at all times
- Insured and currently licensed in good standing with the Ontario College of Pharmacists (Part A)
- Proven ability to foster strong partnerships with other members of the health care team
- Effective organization, planning, and time management skills
- Demonstrated high level of fluency in both written and spoken English
- Knowledge of Kroll Windows application
- Excellent customer service and interpersonal skills, oral and written communication skills and be able to interact with persons of varying levels of professional expertise in a professional and mature manner
- Highly organized and possess excellent time management skills
- Skilled in critical thought processes and be able to prioritize appropriately
- Must demonstrate initiative and creativity
- Self-directed requiring minimal supervision and able to work independently
- Motivated to achieve results that reflect a high standard of quality
- Must demonstrate good listening skills and be adaptable to changes in job environment
- Dependable and responsible in the execution of job duties
- Adaptable to internal work changes
- Knowledgeable with Microsoft Office programs
- Previous Long term care pharmacy experience
